Please bring the completed forms and information as described here.
A parent or guardian must be present.
The consultation will consist of a 45-60 min. interview. Both the parent and the child will be interviewed.
After the appointment a letter will be sent to the referring doctor, summarizing any findings and making treatment recommendations.
